Report a Concern

Types of Reports

健康和心理健康办公室与皇冠博彩社区的所有成员一起工作. 我们的程序使请求帮助和/或记录行为变得容易. 目前对事件报告进行分类的主要有五个类别:

  2. Student Needing Assistance Report
  3. Student Code of Conduct Violation

Responders 是 trained to address every report. 通过健康和心理健康办公室提交的所有事件报告都将根据报告中的信息进行审查并分配给适当的响应者. Concerning behavior should be reported right away.

Examples of concerning behavior 是:

  • classroom disruption
  • self-destructive behavior
  • negative change in academic performance

  • unusual or bizarre behavior
  • incoherent speech
  • intoxication
  • victimization
  • overly stressed
  • unresolved medical issues

Report a Student Needing Assistance (Non–Conduct)

Report an Academic Integrity Violation

This report will be forwarded to the Academic Dean, Associate Dean 和 Instructor of the affected class.

Report a Student Conduct Violation (Non-Academic Integrity)

Report Sexual Misconduct (Title IX)

This report will be forwarded to the Title IX Office.

Sexual Misconduct includes discrimination based on sex, gender identity or gender expression, 和 sexual orientation; sexual harassment; 和 sexual violence, including dating/domestic violence, 跟踪, sexual exploitation, 和 sexual assault.

Title IX Pregnant 和 P是nting Accommodation Request

Confidential Student Ombuds Services (SOS). To file a request for Ombuds Services send an email to   OMBUDS@dljtmp.com  (the email must be submitted by the student requesting the service).

Ombuds Mission Statement

What is an Ombudsperson?

FERPA
